Initial patient evaluations are returned to the physician for review and signature. Progress reports and summation of treatments are sent to the referring on a weekly basis.

Referrals - Monday through Friday, 8 a.m. to 4:30 p.m. Weekends, 8:30 a.m. to 12:30 p.m

Inpatients, outpatients, and outreach patients are seen for an initial evaluation upon referral from the physician. Treatment and goals are established based upon the outcome of the evaluation. Physicians receive copies of the evaluation and monthly progress reports.
