All patients are required to check-in for hospital services including, lab, x-ray, Specialty Clinic, and surgeries.

Park in front of the hospital (the south parking lot off of North Maple Ave.); use the entrance marked Main Entrance. Please have your insurance card, a photo ID and payment for any applicable co-pays and deductibles.
